Although the layout is custom-made, it will still be redistributed on this site as a pre-made layout for others to use after it's finished. This service is also somewhat limited.

Blue Sky Designs' custom-layout service
Cost: $9+ (for the layout and 2 free extra graphics)

Layouts are made for your site and your site only. This service is more flexible and more options are also available. If I'm not available, there are other designers to choose from.
